Once you complete the online information, you will be called back to schedule your first session.
FEES will be discussed when you call and again at the first session. Fees for counseling and mediation will be paid at the time of service. Cash and checks made payable to Family Transitions LLC are the only accepted forms of payment.
INSURANCE claims can be submitted by the counselor for the following health insurance companies that counselor is in network with: Aetna, Blue Cross, Gilsbar, Humana and United Healthcare. Counselor does not accept Medicaid or Medicare. Reimbursement from insurance companies may depend on whether the client has a clinical diagnosis that is covered under the client’s insurance plan. Clients are expected to know their mental health benefits prior to starting counseling. Individuals should call their insurance company to ask about their mental health benefits, including estimated responsibility for outpatient office visits with a licensed professional counselor and any exclusions of their plan. For other insurances counselor is not in network with, documentation of services provided and payment made can be given to the client to submit for possible reimbursement.
